Our Physiotherapist and Occupational Therapist created a personalised rehabilitation programme for one of our clients, Mrs Clark, who was anxious and afraid of falling.

Mrs Jones had been admitted to hospital for eight weeks suffering from jaundice and had several falls at home. She was unable to return home and needed to stay in a care home. The care home manager contacted Therapies on Thames to assess her. She was using a hoist to get out of bed into a chair and was unable to stand or walk. She needed assistance with all her personal care. Mrs Jones wished to return to her house as she had lived in her house since her birth. Our therapists set rehabilitation goals involving Mrs Jones, her family and the care home staff.

Our Physiotherapist worked on her physical rehabilitation and gradually got her walking again with a frame. The Occupational Therapist practised everyday tasks such as washing and dressing. Mrs Jones made such good progress that we could plan to get her back to her home again. Our Occupational Therapist visited her home and arranged the appropriate equipment needed, so Mrs Jones could manage more independently. We then arranged a care package for a carer to come in to help her three times a day, support her with the house work and assist with washing and dressing, toileting and meal preparation. We sent our therapists to her house to encourage her and the carer to manage on their own.
